Overall Responsibility:
Serve as lead organizer and developer of the new Assisted Community Treatment (ACT) program that includes the partnership of internal and external stakeholders from Judiciary and behavioral health systems.
Internal stakeholders include coordinating with team members from multiple departments, such as Homeless Outreach, Street Medicine, and Case Management, to initiate and coordinate petitions and execute orders when they are received.
Provide administrative coordination and ensure program outcomes are met.
Work in collaboration with the psychiatrist(s) and other staff to recognize persons who may benefit from the ACT program, identify mental health needs, affirm diagnoses, and connect to primary and psychiatric care to support medication adherence and improve the quality of life of unhoused individuals.
Essential Functions: Engage and build relationships with persons who are unhoused and exhibit mental illness, and not engaged with a psychiatric provider.
Assist psychiatric providers engage and outreach with these persons to evaluate whether the person’s situation warrants initiating a petition for treatment.
Maintain client rosters and conduct quality assurance of client records and service utilization in coordination with the legal coordinator.
Coordinate such outreach with other HOPE Services outreach efforts including, but not limited to, Homeless Intensive Case Management (HICM) and external organizations conducting outreach services (ie.
Community Care Services (CCS), federally qualified health centers, etc).
Field referrals and outreach requests from community partners, including government, Hawaii County Police Department, other homeless services providers, health plans and private citizens.
Assist with organizing interventions to coordinate the initiation of treatment, including ensuring safe transport to emergency rooms or other sites where the person may be administered medication(s).
Ensure and maintain documentation records to meet standards and be done in a timely manner as required by HOPE Services and funding sources in accordance with all federal, state, and local laws; produce written summaries of individuals who have been identified as eligible candidates for whom to petition the court for Assisted Community Treatment (ACT) or guardianship.
Assist attorneys and medical or psychiatric providers in preparing for court trials and provide testimony as appropriate.
Monitor programs and departments’ progress in meeting the goals and objectives of Assisted Community Treatment.
Ensure program compliance as required by program-specific initiatives.
Other Duties: Reports and attends supervision and team meetings as required.
Document billable activities into HOPE Services electronic health record (EHR) with persons encountered.
Prepare reports for funders regarding ACT program activities.
Oversee petitioning algorithms are followed to ensure legal petitions are filed and executed in a timely manner.
Provide direct service to clients through crisis interventions, counseling, case management, client grievance/dispute resolution, outreach, and other service activities as necessary.
Perform other duties and accept responsibilities as assigned.
Work Environment: This job operates mostly in community-based settings and shelter locations.
Temperatures may vary as working in the field, and the community include varying weather - rain, wind, cold, hot, and humid conditions.
Frequent exposure to stressful situations related to persons served includes but is not limited to domestic violence, ex-offenders, substance abuse, and mental health consumers.
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Supervisory Responsibility: This position is responsible for leading the team and program development.
May have daily supervisory responsibility for in-house community volunteers or interns who are providing project or event assistance.
The responsibility, when assigned, includes ensuring volunteers are working safely and are monitored and supported to have a positive volunteer experience.
Physical Demands: The position requires adaptability, the ability to work under pressure and strong communication skills, resolve conflict, and foster team collaboration.

Experience navigating healthcare, human services, and substance abuse care systems.
CPI certified or able to certify through the organization.
CPR/First Aid/AED certified or able to certify through the organization.
Current Hawaii Driver’s license, preferred clean traffic abstract, and access to an insured vehicle for your use to conduct business.
Scheduling requirements: General business hours are 8:00 a.
m.
- to 4:30 p.
m.
Monday through Friday, but this 40-hour-a-week, exempt position offers flexible scheduling to meet client and program needs.
There will be scheduled meetings (online and in person), and there will be times you are required to work evenings and weekends.
